The most effective way to submit a Print Template request is to send an email to esone.support@deliverhealth.com and include the following:

1. The client code in the subject or body
2. A description of the request
3. "PRINT TEMPLATES" written in the subject line
4. Any relevant attachments - Preferably Word documents with the work needed. 

Unfortunately our current ticketing system will strip out all text formatting (color, bolding, font sizes) from the body of the request, so any of these you would like to show us should be included as attachments in a Word document.

Any new email to esone.support@deliverhealth.com will produce a new ticket. To update an existing ticket, please Reply All to any email involving the previous case. Our ticketing system will scan the Ticket ID in the subject and update the old ticket with the new information.
