An HTML Header or Footer can contain any valid HTML strings. The following codes can be used in a footer:

&p - Current Page Number - Can only use numbers, spelled out will not work
&P - Total Pages - - Can only use numbers, spelled out will not work
[Patient ID] 
[Patient Name] 
[Birthdate] 
[Service Date] 

Example:

         Name: [Patient Name]

         [Service Date]

Result:

         Name: John Doe
