ShadowScribe Template Setup Options for MTSOs

This article outlines the two setups that can be used when implementing a client on ShadowScribe.  It also refers to the Preferred Transcription Format (PTF) which can be Structured or Unstructured.  This is how the transcription or draft will format and be presented to the MT.  For each setup type, Limited Template or Full Template, the PTF describes the impact on the MT working with that type.

Limited template  

Used when you don't want to create a full ShadowScribe template and want the system to put recognized titles in as dictated.

Setup

  • Create ShadowScribe Template with only the titles you need to cover stylesheet formatting. The most common setup is to add a main section like HPI which covers the main section title formatting and a main section with subsections like PE with subs: general, vital signs, HEENT, etc. Be sure to add all possible subsections.
  • Merge Logic is set to As Dictated.
  • Format Rules (stylesheet) set according to client format requirements.

Typing

  • MTs have to type out entire section title and content:
    • PTF = Unstructured: By hand.  MTs responsible for section title formatting and content placement. Content typed as plain text.
    • PTF = Structured: Use Section properties dialog. MTs not responsible for section title formatting and content placement. Content typed inside of content box.

Editing

  • Draft produced in order dictated.
  • If title not recognized or recognized as plain text MTs have to type out entire section title and content:
    • PTF = Unstructured: By hand.  MTs responsible for section title formatting and content placement. Content is edited as plain text.
    • PTF = Structured: Using Section properties dialog. MTs not responsible for section title formatting and content placement. Content is edited within content boxes.
  • Draft Format and Layout
    • PTF = Unstructured.  Presented to MTs as plain text immediately after draft is received with all formatting applied.
    • PTF = Structured.  Presented to MTs with titles and content boxes. No formatting applied.  Formatting applied to upon delivery or if MT does preview with template.

Full template

Used when you want the system to create draft titles in order dictated but merge those titles and content with the document template. In some cases, titles in the raw draft may not be the same titles presented to the MT. This happens when the title code in the draft matches the title code in the template but the template has a different name. Because the draft code matched the template code, the template title is used.

Setup

  • Create full ShadowScribe template with the sections that will be dictated.
  • Merge Logic set to Structured.
  • Format Rules (stylesheet) set according to client format requirements.
  • If PTF = Unstructured, recommended to create traditional document template.

Typing

  • MTs have to type out entire section title:
    • PTF = Unstructured: By hand.  MTs responsible for section title formatting and content placement.
      • If using traditional document template, MTs have less titles to type and only needs to add any titles not dictated.
    • PTF = Structured: Using Section properties dialog, MTs get a list of section titles to choose from, can type a few chars and automatically insert it into report.  MTs not responsible for section title formatting and content placement.

Editing

  • Draft Produced in order dictated with template merging.
  • If title not recognized or recognized as plain text MTs have to type out entire section title:
    • PTF = Unstructured: By hand.  MTs responsible for section title formatting and content placement.
      • No traditional document template will appear.
    • PTF = Structured: Using Section properties dialog. MTs not responsible for section title formatting and content placement.  If title not recognized or recognized as plain text MTs can highlight, words and convert into title.
  • Draft and Layout
    • PTF = Unstructured.  Presented to MTs immediately after draft is received.
      • If using Required Sections, those sections will only show up to MT if..
        • A) The title was recognized and content was merged.
        • B) The title had canned text in it.
    • PTF = Structured. Presented to client upon delivery.  MTs can do preview with template to see final report with formatting applied.
      • If using Required Sections, those sections will always appear to MT.
  • Template merge behavior
    • In some cases, titles in the raw draft would be replaced with titles in the template if they didn't match the raw draft.  This occurs when the code in the draft matches the code in the template. E.g.:
      • Indications in draft.  Indications for procedure in template.  Indications code matches indications for procedure code in template so content is merged in Indications section.
      • History of present Illness in draft.  HPI in template.  History of Present Illness code matches HPI code in template so content is merged in HPI section.
